The World's oldest living former First Lady, Franca Pilla, born on 18 December 1920 in Reggio Emilia, Emilia-Romagna Region, Italy, turned 105 y.o.
She was married to Carlo Azeglio Ciampi who was President from 1999 to 2006.

Presumably that puts her 2nd on the all time list of age achievements of former 1st ladies.
